Patient Forms

Upon entering our office, our receptionist will welcome you as a member of our family. We will request that you complete our patient forms located in the New Patient Center. This paperwork provides us with your health history and information on your condition. If you are unable to bring these forms with you on your first visit please arrive 20 minutes earlier to complete the paperwork.


Consultation

Next, you will have a 15 minute no charge consultation with the doctor to discuss your health-related problems and concerns. This initial visit is designed to learn more about you, your condition, and expectations to determine if chiropractic care can meet your goals. If the doctor decides to take your case all associated fees will be discussed before services are provided.  You are responsible for these fees following any services provided.


Health History and Examination

After your consultation, and if the doctor agrees to take your case, the doctor will take a detailed health history and perform a complete chiropractic examination. This will include testing your spinal range of motion, as well as other standard orthopedic, neurological, postural, and physical examinations. The doctor will gain valuable information regarding your condition so they have a better understanding of how chiropractic can help you. If necessary, the doctor may take X-rays or refer you for additional testing procedures. However, nothing will be done in our office without your prior consent.


X-Ray Studies

Depending on your specific condition, we may take X-rays as well. X-rays help us develop the most effective treatment plan for you, and alert us of any serious spinal conditions. Most people are amazed once they see their X-rays up close. Our office utilizes on site state of the art digital X-ray.


Report of Findings

Once the information is collected and examinations are performed, you will return for your second office visit.  There is no additional charge for your detailed report of all findings of your condition.   The doctor will  answer any questions including:
1. How can you help me?
2. How often do I need to come in?
3. What will my treatment cost?


Treatment

At the conclusion of your second visit report of findings, you can choose to experience your first treatment. This may include spinal adjustments, manual therapy, therapeutic exercise, and/or soft tissue massage. Treatment is interactive, so you can express concerns about the different styles of treatment at anytime.


Home Support Care

After your first treatment, the doctor may suggest home support care to incorporate outside of treatment. If you are in pain when you first come into our office this may include: ice or heat application instructions, certain activities or positions to avoid, and at home exercises and/or stretches. The purpose of home supportive care is for you to achieve good spinal alignment, have a healthy diet, exercise, and maintain a positive mental state.
